Computer Donation Program Application

Basic Information about the Program:

To apply for a computer scholarship, please see the form below.  All of the following must be filled out and turned in for consideration.

The program is ongoing, and computer scholarships will be given year round as supplies last.  Although open to all Illinois 4-Hers, preference will always be given to McHenry County 4-Hers.
